Navigator helping a family find local resources
We hear you

Why this matters.

There's no shortage of dementia resources. There's a shortage of someone who can tell you which ones are worth your time, accept your insurance, and have openings this month.

  • You've started a dozen searches and finished none.
  • Every program has different paperwork and wait times.
  • You don't know who's reputable and who isn't.
  • By the time you find something, the need has changed again.
How we help

HōttoCare is there for you.

Your navigator maintains a vetted network of local providers — home care, adult day, transportation, legal, financial, faith communities, and more. We match you to what fits and make the warm handoff so you're not starting cold.

What's included

Everything you get with this service.

  • Vetted local provider matching (home care, day programs, more)
  • Insurance and Medicare benefits navigation
  • Legal and financial referrals (estate, conservatorship, Medi-Cal)
  • Transportation coordination
  • Faith-based and community group connections
  • Application support and paperwork help
How it works

Simple, supportive, at your pace.

  1. 1

    Tell us what you need

From a home health aide to a dementia-friendly attorney — describe the gap.

  2. 2

    We do the research

Your navigator surfaces 2–3 vetted options that fit your situation and budget.

  3. 3

    Warm introductions

We make the call, send the email, or set up the meeting on your behalf.

  4. 4

    Stay connected

We follow up to make sure the connection worked — and pivot if it didn't.

Common situations

Caregivers come to us when…

You need help at home

Time to bring in in-home care but you don't know which agency to trust.

Legal questions

Power of attorney, conservatorship, or estate planning is overdue.

Medi-Cal or VA benefits

You suspect you qualify for more than you're receiving.

Transportation gaps

Getting to appointments has become its own full-time job.

"Thank you for helping me look over the mail I get for my grandmother. It can be hard to distinguish which forms are important and which are junk mail. I appreciate having the HōttoCare team to contact when these things get confusing."

Questions, answered

Frequently asked.

Do you have providers in my area?

+

Our deepest local networks are in California (Bay Area, LA, and Central Valley), and we're growing nationwide. Wherever you are, we also offer virtual resources, national directories, and guidance you can use anywhere in the U.S.

Do you take a referral fee?

+

No. We're funded through Medicare's GUIDE Program, so our recommendations are based on fit — not commissions.

What if a provider doesn't work out?

+

Tell us. We re-match quickly and use the feedback to improve our network.

You don't have to navigate this alone

We can help guide you through it.

A short, no-pressure conversation with a HōttoCare navigator is often the most hopeful thing a caregiver does all month.
