Meet the People Behind HōttoCare
HōttoCare was created to help families navigate dementia care with greater confidence, support, and peace of mind. Our physicians, social workers, care navigators, and support team work together to provide guidance throughout the dementia journey.

Why we created HōttoCare
Families navigating dementia often face overwhelming decisions, fragmented care systems, and a lack of coordinated support. HōttoCare was created to provide the guidance, education, and ongoing support families need throughout the dementia journey.
Our founder, Dr. Yumi Taylor, spent years caring for older adults across hospitals, rehabilitation facilities, home visits, nursing homes, free clinics, and primary care practices. Through those experiences, she saw firsthand how difficult it was for families to coordinate care and access support.
After additional training at Stanford and healthcare innovation leadership at Sutter Health, she founded HōttoCare to build a more connected model of dementia care — one that supports not only the person living with dementia, but also the family caregivers supporting them every day.
"We started HōttoCare to fill critical gaps in homebound care—but we needed a sustainable model. When Medicare's GUIDE program launched, it was our 'Eureka!' moment. Finally, a solution that values dementia caregivers and makes financial sense for providers. We knew this was the future of care."

What families tell us most often.
"I didn't feel heard by the teams in the hospital—the doctors and social workers. It was so helpful for the HōttoCare physician to step in and give us a voice."
"When I have questions about how to manage different situations, I reach out to HōttoCare, and you know how to handle it—or where to guide me. This saves me so much time trying to figure it out on my own. Thank you!"
"It was nice to participate in a learning session with families going through the same thing as me. I want to affirm and uplift other caregivers like myself to keep a positive outlook and know that there is support out there. Thank you, HōttoCare, for making these sessions accessible."
"Thank you, HōttoCare, for paying attention to the details, being kind, and being so skilled at supporting people with dementia."

What our partners say about working with HōttoCare.
“A patient has been under my care at the Stanford Center for Memory Disorders since June 2024 and has a diagnosis of Lewy-body dementia (LBD). Since starting services with HōttoCare, his care partner has noticed a positive outcome and improvement in his mood and well-being. I encourage his continued participation in this program and support their request for additional hours each week.”
