What is HōttoCare?

HōttoCare is a physician-led dementia care and caregiver support organization. We combine a multidisciplinary clinical team — doctors, nurse practitioners, social workers, navigators, and care assistants — with practical caregiver support. The name 'Hōtto' is Japanese for 'a sigh of relief,' which is what we want every family to feel when they find us.
How is HōttoCare different from a typical home-care agency?

We're not a staffing agency. We're a coordinated care team that wraps around your family — handling clinical care, caregiver coaching, benefits navigation, and respite coordination together, not in silos.
GUIDE Program & Cost
This sounds too good to be true. What's the catch?

There isn't one. Medicare created the Guiding an Improved Dementia Experience (GUIDE) Model because better dementia support at home reduces costly hospitalizations and ER visits. When eligible families get coordinated care, everyone wins. Medicare pays us, so you don't.
Is HōttoCare really free for eligible families?

Yes. For families enrolled in the GUIDE Program, comprehensive dementia care navigation, caregiver coaching, 24/7 helpline access, and respite support are covered by Medicare at no cost to you.
What is the $2,500 respite benefit?

Eligible caregivers in the GUIDE Program can access up to $2,500 per year for respite care — in-home help, adult day programs, or short-term stays. Your navigator helps coordinate it and handles the paperwork.
Eligibility
Who qualifies for the GUIDE Program through HōttoCare?

Your loved one needs a diagnosis of dementia or significant cognitive decline, must be enrolled in Original Medicare (Parts A & B), must live in the community (not a long-term care facility), and must be a California resident. We can help confirm eligibility on a short call.
What if we're not in California — can HōttoCare still help?

Yes. Our educational resources, peer groups, FAQs, and care navigation guidance are available to caregivers across the United States at no cost. GUIDE Program clinical services are currently limited to eligible California residents.
What if my loved one is on Medicare Advantage?

Medicare Advantage members are not currently eligible for GUIDE. We can still help you with education and connect you to resources. Some families switch to Original Medicare during open enrollment to access GUIDE — we can walk you through the tradeoffs.
What to Expect
How quickly can someone help me?

Most caregivers hear back from a care navigator within one business day, often the same day. If you're in crisis, tell us — we'll prioritize you.
Do you replace my loved one's doctor?

No. We work alongside your existing doctors and specialists, coordinating care and translating between them — not replacing them.
What does ongoing support look like?

Scheduled check-ins, 24/7 access to a helpline, caregiver coaching sessions, a personalized care plan that adjusts as needs change, and proactive outreach when things shift.
