The GUIDE Model is a Medicare initiative designed to improve care for people living with dementia while supporting the family members who care for them. Most families navigating dementia describe the same thing — fragmented care, burnout, and not knowing what to do next. GUIDE replaces that with one trusted team.

How much does the GUIDE Program cost?

For eligible Medicare beneficiaries, GUIDE services through HōttoCare are 100% covered — there's no out-of-pocket cost to the family.

Who is eligible?

Adults with an Alzheimer's or related dementia diagnosis, enrolled in Medicare Part A and B, and currently residing in California. A short call confirms eligibility.

What is the respite benefit?

Eligible caregivers can access up to $2,500 per year in qualifying respite services — used for things like in-home aide hours or adult day programs so you can rest.

Do I have to switch doctors?

No. Your navigator works with the doctors and specialists you already trust — coordinating around your existing care team, not replacing it.

What if I don't live in California?

GUIDE clinical services require California residency, but our educational resources, peer support groups, and guides are available to caregivers nationwide.
