Care Navigator
You don't need a background in healthcare. You need to know what it's like to sit with a family in the hardest chapter of their lives — and to want to help the next one find their footing faster than you did.
Why this role exists
Every month, more families come to HōttoCare looking for one thing: a person who will walk with them. Not a hotline. Not a portal. A guide. Our Care Navigators are that person — the one a daughter calls at 9pm because she doesn't know what to do next, and the one who calls back.
We're building our team of Navigators one relationship at a time, and we're hiring on an ongoing basis. If this sounds like the work you've already been doing for free — for your own family, your neighbors, your community — we'd love to meet you.
Who we're looking for
Warm, coachable, and growing.
You're warm before you're polished. We can teach systems and process; we can't teach someone to care.
No specific background required.
Navigators come from all kinds of work — retail, grocery, hospitality, teaching, family caregiving, and more. What matters is how you show up for people, not your resume.
A personal connection to dementia caregiving.
You've cared for, or cared alongside, someone living with dementia — as a family member, a friend, or in your community. You understand the exhaustion and the love, because you've lived it.
In it for the long run.
This isn't a stepping-stone role for you. You're looking to build something meaningful with families over the long haul — ideally five years or more.
Reliable under weight.
You can hold a hard conversation with steadiness, follow up on a hundred small details without dropping any of them, and know when to ask for help.
What you'll do
- You'll be the single point of contact for a caseload of families, guiding them through the GUIDE Model from first call to steady footing.
- You'll help families understand what they're eligible for and take the next concrete step — no jargon, no runaround.
- You'll check in regularly, catch problems early, and adjust the plan as a family's needs change.
- You'll go through paid training as part of onboarding, and keep learning and growing with the team.
