You keep the clinical relationship, and HōttoCare will:

- Diagnose dementia
- Manage medications
- Address social needs
- Share updates with you

We offload your inbasket

We’re available to patients and caregivers by text & phone, so we can address many concerns before they hit your inbasket.

We support caregivers

We provide dementia education, offer peer support, and help caregivers get a $2,500 yearly allowance for respite care to improve caregivers’ resilience.

We’re 100% covered by Medicare

HōttoCare is part of the Medicare GUIDE program, fully-covered by conventional/FFS Medicare with no cost to the patient.

Who Qualifies?

Patients who:

  • Are experiencing cognitive decline or who have a dementia diagnosis

  • Have original/traditional Medicare with or without Medi-Cal

  • Live at home, assisted living, or personal care home (not a nursing home)

  • Are not enrolled in hospice or PACE
    program

  • Reside in Northern California (Their caregivers may live anywhere)
